Memory Assessment Services

Memory assessment services are located across Lancashire and aim to comprehensively assess anyone of any age who might be worried about their memory.

The teams are made up of nurses, doctors, occupational therapists and psychologists. Anyone can be referred via their GP, or other hospital specialist because of concerns.

Chorley and Preston Hospitals

Chorley & South Ribble Hospital and Royal Preston Hospital offer dementia-friendly support for appointments and stays. This includes a Fast Track system for quicker clinic access and extra help from Dementia Champions.

Contact PALS on 07591 988962 or email PALS@lthtr.nhs.uk at least three days before.

They also follow a Carers Charter, encouraging open visiting to help familiar faces support eating, drinking, and comfort. Bringing a hospital passport, This Is Me booklet, or completing a Forget Me Not form helps staff provide personalised care, especially if communication is difficult.

Dementia Community links

Community Links staff visit people in their own homes to provide information and advice in regards to memory loss, the symptoms of dementia and how to seek help. We will connect you into a network of providers to suit your needs.

Community Links is a free service and offers short term support, both on a one-to-one basis and through group settings. Its aim is to reduce social isolation, promote independence and to boost physical, social and emotional wellbeing. It also helps to build confidence through linking people into positive activities and community settings.

The Lancashire Carers Service

Comprehensive support to unpaid adult carers across Lancashire. Services include Carers’ Assessments, emergency planning, emotional support through the Carers Help and Talk (CHAT) Line, peer support groups, befriending services, and access to wellbeing resources.

Carers can also participate in the Carers’ Community Network to connect with others and share experiences.

Age UK Lancashire Dementia Services

Age UK Lancashire provides holistic support including therapy groups, education sessions, and activities to help people with dementia stay active and socially connected. They also offer tailored advice and emotional support for carers.

Alzheimer’s Society

Alzheimer’s Society is here for you through our Dementia Adviser service. In South Ribble, we work with the NHS to provide post-diagnostic support. After a diagnosis, we’re available whenever you need help understanding what’s happening, what support is available, and what to do next. We can visit at home, meet in the community, or talk by phone or email.

Our local Dementia Advisers live and work in Central Lancashire. As the UK’s leading dementia charity, Alzheimer’s Society supports people with dementia, their families, and carers through expert advice, a support line, and a wealth of online resources.

Talking Therapies

Lancashire & South Cumbria Talking Therapies

Lancashire and South Cumbria Talking Therapies offers free mental health support for adults, including carers of people with dementia and those in early stages of the condition. Through counselling, CBT, and guided self-help, the service helps manage stress, anxiety, and low mood, promoting better emotional wellbeing. Support is available online, by phone, or face-to-face, with easy self-referral.
